Continuum Health Alliance, LLC Data Breach Investigation

Continuum Health is a health management company based in New Jersey. Between October 18, 2023 and October 19, 2023, Continuum Health experienced a network breach that was discovered on October 19, 2023. After an investigation was completed on March 8, 2024, it was revealed that individuals’ personal identifiable information and protected health information was breached including first and last names, dates of birth, social security numbers, health insurance information, medical information, and phone number. Notification of the breach was sent to affected individuals on or about April 29, 2024.
